GENERAL AFFADAVIT
State of Florida, County of Gadsden ss.
In the Matter of Pheba A. Church, widow of Uzziel B. Church, Co. D. 11th Indiana Inf. wid. Orig. #1,082,715.
ON THIS 27th day of August, A.D. one thousand nine hundred and seventeen, personally appeared before me, a notary public, within and for the County and State aforesaid, duly authorized to administer oaths, Mary Wells, aged 68 years, a resident of No. [blank] Street, [blank], County of Gadsden, State of Florida, well known by me to be reputable and entitled to credit, and who, being sworn according to law, declares in relation to the aforesaid case as follows:
That affiant is a sister of said claimant and has personal knowledge of the fact that said claimant and said soldier were married near Perry, Ralls County, Missouri, March 24, 1867.
That said affiant and said claimant were living with their parents in Ralls County, Missouri at the time of said marriage, and that said affiant was present at said marriage.
I certify that I am not interested in the prosecution of this case. My post office address is Havana, Fla.
